The successful candidate will be responsible for assessing the needs of clients, developing individualized plans of care, and providing resources and referrals to help clients achieve their goals.
The ideal candidate will have excellent communication skills, a passion for helping others, and the ability to work with a diverse range of individuals.
Responsibilities:
Provide emotional support to individuals and families in need.
Assess the needs of clients and develop individualized plans to meet those needs.
Conduct home visits to evaluate the safety and well-being of clients.
Provide crisis intervention services when needed.
Refer clients to appropriate community resources and services.
Document all client interactions in a timely manner.
Maintain professional boundaries with clients.
Attend meetings and trainings as required.
