Per Diem Hospice Field RN
Victorville, CA Victorville, CA Per diem Per diem $48 - $52 an hour $48 - $52 an hour Coverage area includes Barstow, Phelan, Adelanto, Lucerne Valley, Oak Hills, Hesperia, and Victorville.
The RN coordinates hospice care with other team members and patients/families to ensure that the patients' needs are being met.
The Case Manager consults with the attending physician and Hospice Medical Director concerning changes in patients' conditions requiring order changes and/or changes in the plan of care.
Responsibilities:
Completes an initial, comprehensive and ongoing assessments of patient and family Oversees Plan of Care for accuracy and appropriateness Prepares discharge summaries Coordinates and conducts IDG meetings Oversee clinical supervision of nursing personnel; LVN's and Home Health Aides Supervise the maintenance of medical records assuring accuracy, completeness and compliance Explains nursing and other Agency services to patients and families as a part of planning of care Adheres to Federal, state and accreditation requirements including Medicare and Medicaid regulations Completes own nursing visit notes as well as audit other nursing staff notes Required
Qualifications:
At least 1 year experience in Hospice within that last 3 years Current California RN license Excellent written and verbal communication skills Valid CA Driver's License and auto insurance Current CPR Current TB/Physical Pass Background Clearance Preferred
